MEHT Staff - Opt Out Online Form

As an employee of MEHT, you will automatically become a member of the Trust once it achieves Foundation Trust status. 

If you do not wish to become a member of the Trust, you must ‘opt out’ of membership. 

In order to ‘opt out’, please complete the following online form:
