Diabetes Service

Diabetes Care service profile

 

The hospital based specialist diabetes team provides a service for patients with Type I and Type II Diabetes Mellitus, on both an in- patient and an out-patient basis.

 

  • Patients benefit from a comprehensive service that includes Diabetologists, nurse specialists, specialist dieticians, podiatry and footwear

  • Paediatric diabetes services are currently based at St John’s Hospital, although we maintain close links with this service.

  • The joint diabetes / obstetric ante-natal clinic is held at St John’s Hospital and will continue at this location until such times that maternity services re-locate to the Broomfield site upon completion of the new Hospital.

Referrals

 

Referrals to the diabetes clinics should be made via the GP to:

 

Dr Jeremy Fletcher

 

  • Type 1 patients

  • Any Patients with diabetes related complications including diabetic nephropathy


Dr Alan Jackson

 

  • Foot care

  • Ante- natal clinic


Dr David Cunnah

 

  • Patients with diabetes living in the Braintree, Halstead & Witham area


Diabetes Specialist Nurses (DSN)

 

For assessment and education

 

  • For out-patients via letter or fax from GP or Practice nurse)

  • For in-patient care ( via ordercomms)


The DSN team and special interest areas are as follows:

 

  • Jane Young  : Lead Nurse and Renal diabetes (Haemodialysis, PD)

  • Margaret Bardle : Pre-conceptual and ante-natal care

  • Lisa Joslin : In-patient care with an interest in Erectile dysfunction

  • Hazel Hynes : Adolescents and young people with Type 1 diabetes

  • Agnieszka Graja : In-patient care and genetic diabetes

 

 

Nutrition & Dietetics


For assessment and education

 

  • For out-patients via letter or fax from GP or Practice nurse


The Nutrition & Dietetic Team and special interest areas are as follows:

 

  • Pauline Bird (Clinical Manager for dietetics), special interests in pre conceptual, ante- and postnatal care

  • Chris Cheyette, lead for Type 1 education (BERTIE) and specialist in weight management

  • Bridget Lee, lead for Type 2 education, and specialist in weight management

 

 

Podiatry

 

  • Eleanor Swan (Lead), Janice Armario and Sunil Patel

  • Referral for in- patient  diabetes related foot-care is via the ordercomms system or for toe nail service (for patient who have extended admissions only) by fax to diabetes centre

  • Out- patient referral via the community podiatry clinics depending on locality

Other services include:

 

  • New patient diabetic assessment, Consultant and/or senior DSNs

  • Telephone advice for admission avoidance and treatment titration

  • Telephone advisory service for patients and Primary care health care professionals.

  • Email to diabetes nurses (DSNs) for current patients

  • Education programme for Type II patients, including insulin conversion

  • Education  programme for Type I patients with diabetes, including insulin initiation

  • Continuous  Blood Glucose monitoring

  • Education for both primary and secondary staff
